THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Ascend Wellness Cooperative and the licensed providers who serve patients through our platform are committed to protecting your Protected Health Information ("PHI"). This Notice is subject to legal review.

Our Duties

We are required by law to maintain the privacy of your PHI, to provide you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

How We May Use and Disclose Your PHI

  • Treatment: to provide, coordinate, and manage your telehealth care, including sharing information with the providers and pharmacies involved in your care.
  • Payment: to obtain payment for services, including processing your medical-review fee and medication orders.
  • Health Care Operations: to support quality, safety, administration, and business operations of our platform.
  • As Required by Law: when federal, state, or local law requires disclosure, including public-health and safety situations.

Uses and Disclosures Requiring Your Authorization

Most uses and disclosures not described above — including marketing that involves your PHI and any sale of PHI — require your written authorization, which you may revoke at any time in writing.

Your Rights

  • To inspect and obtain a copy of your PHI;
  • To request a correction (amendment) of your PHI;
  • To request restrictions on certain uses and disclosures;
  • To request confidential communications by alternative means or location;
  • To receive an accounting of certain disclosures;
  • To receive a paper copy of this Notice on request;
  • To be notified following a breach of unsecured PHI.

With your consent, we may communicate with you about your care by email and SMS. These channels carry inherent risks; you may opt out at any time (reply STOP to texts).

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at the contact below or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to This Notice

We may change this Notice and make the new terms effective for all PHI we maintain. The current Notice will be posted here with its effective date.
