These disclaimers apply to all information and services provided through Ascend Wellness Cooperative. This document is subject to legal review.

1. Telehealth Consent

Our services are delivered through telehealth, meaning a licensed provider may evaluate you and make treatment decisions based on the information you submit electronically, without an in-person visit. Telehealth has benefits and limitations; by proceeding you acknowledge that a provider may determine that an in-person evaluation is necessary and may decline to prescribe.

2. No Guarantee of Treatment

Completing the questionnaire and paying the $119 review fee does not guarantee that you will be prescribed any medication. Licensed providers exercise independent clinical judgment and may decline treatment when it is not appropriate or safe.

3. About GLP-1 Medications

Semaglutide and Tirzepatide are prescription medications. They are not suitable for everyone and carry risks and possible side effects, which commonly include nausea, vomiting, diarrhea, constipation, and abdominal discomfort, and may rarely include serious effects. These medications are contraindicated for individuals with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, and for individuals who are pregnant or breastfeeding, among others. Discuss all risks with your provider and review the medication's prescribing information.

4. Results Vary

Individual results vary. Weight-management outcomes depend on many factors and are most effective when combined with healthy nutrition and physical activity. No specific result is promised or guaranteed.

6. Compounded Medications

Where medications are compounded, they are prepared by a licensed pharmacy pursuant to a valid prescription. Compounded drugs are not FDA-approved products; your provider will discuss whether a compounded option is appropriate for you.

7. Independent Providers & Pharmacies

Ascend is a technology platform. Medical services are provided by independent licensed providers, and medications are dispensed by independent licensed pharmacies. Ascend does not practice medicine or pharmacy.
